The COVID-19 pandemic has presented several challenges for pharmacist-run anticoagulation clinics. In this 2020 Midyear Clinical Meeting presentation, Yale New Haven Pharmacists share their approach to adjusting clinic in-person visit workflows to continue to provide high-quality patient care and monitoring while adequately protecting patients and staff. They describe their remote methods to monitor anticoagulation therapy and identify their successes with intradepartmental communication.
