Educate (Teaching point) What is the CONTINUUM OF CARE? It is beyond the end of life, consider PRIOR to that: Aging in place at home, or move to RCFEs, home care, mobile physicians, transportation, home health for physical therapy (after a fall and hospitalization), palliative care, hospice.
