For myself, I've just been busy as an attending and then also just trying to make sure that my 11-month-old doesn't harm himself in any way because he's a pretty mischievous kid.
All right, with that, Mukund has a case today.

So I'm going to just summarize for the audience of what I think is relevant from the first aliquot.
We're picking up two timeframes early in this case.
One is the sudden right upper quadrant pain, and then the slower story of distension, dyspnea, and the 45-pound weight loss.
suggesting a strong clue that already that this is going to be a complex case.
Now, my second thing for us to ponder over is before we anchor on the gallbladder, what unifying diagnoses could possibly connect your right proclinic pain, new ascites, and major weight loss?
If you guys are thinking that the unifying diagnosis would be a hepatobiliary infection or inflammation such as cholecystitis or cholangitis, that's correct.
Second, a vascular liver process related to his JAK2-positive myeloproliferative neoplasm.
That would be also a good, including splanchnic thrombosis or portal sinusoidal vascular diseases.
That's also a great consideration.
