So does a low SAG hemorrhagic inflammatory ascites profile fit with the story we were telling up to now?
Or does it force a new framework?

I would argue that it forces a new framework.
Up to this point, we were asking whether portal hypertension explained the ascites.
A SAG of 0.6 says that it cannot be the main explanation here.
The hemorrhagic and inflammatory profile means we now need to think about peritoneal irritation or invasion, not just pressure-driven fluid accumulation.

But I think I'll ask this one question for the audience here.
How do you reconcile mild portal hypertension with an acidic profile that looks very non-portal hypertensive?

An HVPG of 7 means portal hypertension is present, but it is likely mild and clinically not that significant.
