Thank you so much for staying on for some extra content on Doctor's Notes. Thank you for having me. Have you ever had a moment in your life where you sort of suddenly realised that balance was important? Yes, when I've been trying to learn to ski as an adult. I have a partner who's a very good skier and really wants me to be able to ski. I mean, I'm just not very good and I don't enjoy it. I feel completely mentally exhausted from the whole task of trying to balance.
To learn a skill that high level as an older adult is challenging. My son, who lives in Oslo, we've tried cross-country skiing as well, which was very interesting. My partner, who's a good downhill skier, was actually worse at that than me.
Because I've tried cross-country skiing and it's actually very hard to balance. You're on these long skis and you're on slippery snow and ice. It's potentially harder than downhill, I think, because you're on very thin skis. In order to slow down, if you're going downhill, you have to balance on one side, lift up your foot and snowplow with one ski. So it's really difficult as well. So definitely I've become aware of how important this balance is. Tell us about how you got into doing vestibular rehabilitation.
Muistan aiemmin olevani lektuuriin, jossa he nÀyttivÀt meidÀt labyrinttiin. Se oli pieni kuin sinun pientesi. Niin kuin anatominen spesimen? KyllÀ. Semisirkulaiset kanalit. En usko, kuinka pienet ne ovat. En tiedÀ, kuinka iso ne ovat. MitÀ ne nÀyttÀvÀt? Ne sopivat pientesi? Sinun pientesi.
The tip of the little finger? So tiny. You can see it though, can't you? Just about, yes. But I just remember looking at it and thinking, wow, something that small keeps us balanced. And it's just integral to our function. Is it like the size of a pea or smaller? Probably about the size of a pea. I love that.
KyllÀ, se on balanssipÀÀtÀ. BalanssipÀÀtÀ voidaan olla metsÀpÀÀtÀ. KyllÀ, Chris, se on tÀysin erilaista. BalanssipÀÀtÀ. KyllÀ, se on oikein. Maggie, mitÀ meidÀt voidaan tehdÀ? Me tehdÀÀn testi sensori-integration for balance. Se on kutsuttu klinikallisen testin sensori-integration for balance. TÀmÀ on testi, jota me voidaan tehdÀ hyvin tai pahasti.
Yes, yeah. Perfect. So it gives me or us physios an idea of how well your brain is tuning into your vestibular input by narrowing down other sensory inputs. So how does the test work? So we start by doing feet together on the floor. So if you bring your feet, do you want to do it now? Should we do it together? Can we do a bit of it we can do together? You can do this part together. Okay, here we go. Feet together. Feet together.
Sitten katsotaan, kuinka valmistuminen on 30 sekuntia lÀmpenemisessÀ, josta pitÀisi olla hyvÀ, koska sinulla on visuaalinen tuntemus ja ympÀristön tunnistaminen. Olemme tehneet vÀhentÀmÀÀn tuntemuksen ympÀristön tunnistaminen ympÀristön tunnistaminen ympÀristön tunnistaminen ympÀristön tunnistaminen ympÀristön tunnistaminen ympÀristön tunnistaminen
If I was in one of the guards regiments and I spent a lot of time doing this, would I stop swaying? I mean, when you look at the soldiers at Buckingham Palace, they've mastered, I mean, they've got very minimal sway, haven't they? And they're wearing big hats quite often. Yeah, I was going to say the big hat, it must be harder to do in a windy day with a big hat. Actually, it helps. So you're both looking like you've got a normal response to this sensory condition. Okay. Okay, I'll go first. Here we go. You come onto the balance pad.
So this is a bright blue piece of foam. And you want your feet close together. Feet close together, okay. So this is reducing sensory input, so the proprioceptive input that you get from your ankles and feet, it's reducing it because it's a soft surface. Okay, so you feel quite comfortable with eyes open. I do feel okay with this, yeah.
Okay, so now close your eyes. Okay, shush everyone. He's swaying. There is more sway. So this really narrows down the input available to your vestibular input from your inner ears. Because we've taken away visual and we've narrowed down, we've reduced input from your feet.
TÀmÀ on normaalia rangaista.
Zahn has that degree of concentration and focus now. My whole brain is working to do this. How am I doing, Maggie? You're doing really well. Now, do you do it on one leg? No, because that brings in your saving reactions. And with this test, we're looking at how comfortable your brain is to tune into your vestibular input. You're looking very good level. But we should be, shouldn't we, if we sort of...
I mean, it's definitely, if you're wanting to improve your balance, this is one way you can do it for sure. And so often, you know, elite sports people that come through to the concussion clinic, they often need to work on this area if they've had a concussion. And I'll often say to them, you know, your play could be even better than it was before if you work on this, because if you haven't been working on it, it's sort of the icing on the cake. Because there's always ways that you can improve it and really fine-tune your system.
With physical literacy in kids, you know, kids now spend more time on screens, they roam less, they use their bodies less. Tell us about that with regard to balance. You know, balance is something that you have to nurture and stimulate by different activities. And the broader range of activities that a child can do, the more beneficial for their balance. And, you know, I think...
