There's also this really cool mechanism of an aortic dissection presenting with high vagal tone from the abrupt rupture.
I don't have a way of reconciling the aortic dissection with the EKG apart from when I shared the idea that Takotsubos could be at play.
The deeper concept in Takotsubo is whenever you have those T wave changes, it's just a marker of sympathetic excess on the heart.
And so you can easily imagine why somebody with an aortic dissection has a sudden onset sympathetic excess in their heart.
But, you know, this is now a very, very serious issue.
And the patient definitely needs a CT scan.
And now what's changed with the CT scan is I'm prioritizing an aortic dissection protocol as opposed to a PE protocol.
And the good news is that you'll still get most of the main and segmental branches.
You might not get sub-segmental PEs, which are probably okay in a scenario like this.
