Too often what happens is we blame the person, and the person blames themselves, and that compounds their feelings of worthlessness and difficulty, when really we are asking people to navigate systems that are incredibly broken, fragmented, siloed. Okay, the person's released from jail, then what? Discharged from an inpatient facility, then what? How do we get them from that to a home in the community where they can live and engage and live a life? And we know that people can do well if they can get access to those services.
