Single-Dose Liposomal Amphotericin B Treatment for Cryptococcal MeningitisCitationNew England Journal of Medicine, 2022; 386(12): 1109–1120論文の要約本試験は、サハラ以南の5カ国でHIV陽性成人を対象に実施された第3相ランダム化非劣性試験である。以下の2群を比較した。単回高用量リポソーム型アムホテリシンB（AmBisome）群：初回に10 mg/kgを点滴し、その後14日間フルサイトシンおよびフルコナゾールを併用。WHO標準治療群：7日間のドコシコレート型アムホテリシンB＋フルサイトシン、続いて7日間フルコナゾール。10週時点の全死亡率は、AmBisome群で24.8%、標準治療群で28.7%、差は –3.9ポイント（非劣性マージン内）。脳脊髄液中の真菌除菌速度も両群で同等だった。副作用（グレード3/4）はAmBisome群で50.0%、標準群62.3%と、AmBisome群の方が少なかった。臨床的意義単回投与のAmBisome併用療法は、従来の強毒性治療と同等の効果を示しつつ、副作用が少なく、安全性と利便性を兼ね備えた治療法である。資源制限地域における実臨床への導入価値が高い。
