The United Kingdom meningococcal vaccine (4CMenB) programme against gonorrhoea: a review of the evidence and knowledge gapsCitationJournal of Infection, 2025; in press概要2025年8月、英国は世界で初めて淋菌感染症予防を目的とした髄膜炎菌ワクチン（4CMenB）のターゲット接種プログラムを開始した。対象は、前年に細菌性STIを経験した、または直近3か月で5人以上の性的パートナーを持つなど高リスクと判定された、ゲイ・バイセクシャルおよびその他の男性間性交渉者（GBMSM）である。観察研究により、4CMenBは淋菌感染に対して約30〜41％の交差防御効果を示すと推定されている。プログラムは性的健康クリニックで他のワクチン（A型肝炎、B型肝炎、HPV、mpox）と併せて実施され、接種効果・持続期間・感染部位別有効性・抗菌薬耐性株への効果などを全国サーベイランスで評価する。重要な課題として、自然感染後に免疫が成立しにくい淋菌に対してワクチンがなぜ一定の防御効果を示すのか、感染既往や曝露頻度がワクチン効果に与える影響、無症候性感染や部位別効果、行動変容の影響などが挙げられている。4CMenBは完全な感染阻止はできないものの、疾患負荷や抗菌薬使用量の減少に寄与しうる。さらに、髄膜炎菌感染症や性感染経路を持つN. meningitidis株に対しても防御効果が期待される。英国で得られる実地データは、他国が同様のプログラム導入を検討する際の重要なエビデンスとなることが期待されている。
