Effect of C-Reactive Protein–Guided Antibiotic Treatment Duration, 7-Day Treatment, or 14-Day Treatment on 30-Day Clinical Failure Rate in Patients With Uncomplicated Gram-Negative Bacteremia: A Randomized Clinical TrialCitationJAMA, 2020; 323(21): 2160–2169概要スイスの3施設で実施された多施設無作為化比較試験。非複雑性グラム陰性菌菌血症の成人504例を、以下の3群に無作為化し比較した。CRPガイド群：CRPがピークから75％低下し、48時間解熱した時点で抗菌薬中止固定7日群：7日間の抗菌薬治療固定14日群：14日間の抗菌薬治療主要評価項目（30日臨床的失敗率）CRP群：2.4％7日群：6.6％14日群：5.5％いずれも14日群に対して非劣性が確認された。副次評価項目90日時点の失敗率も群間差なし治療期間中央値はCRP群と7日群が7日程度、14日群は14日C. difficile感染は長期治療群で多い傾向有害事象や死亡率に差はなし結論非複雑性グラム陰性菌菌血症において、CRPガイド下治療や7日間治療は14日間治療と比較して非劣性であり、抗菌薬曝露の短縮が可能であった。CRPガイドは適正使用の一助となる可能性がある。
