It turned out that Smith had no formal training as a forensic pathologist.
Canada was way behind the rest of the world in creating rules and standards for the field.
Smith's false and misleading testimony led to many wrongful convictions.
One man spent 12 years in prison for the death of his four-year-old niece.
It turned out she died of natural causes.
The fallout was enormous.
It led to a public probe known as the Gouge Inquiry.
One of its key findings was that coroners and forensic pathologists should stop thinking dirty.
To think dirty means to look at a case through a lens of suspicion, to start from the assumption that a crime may have been committed, and then look for evidence to support it.
The message after gouge was clear.
