I thought it was a head-on crash or somehow.

I just assumed the shoulder straps broke the collarbones.
But apparently he went to the side to the left side and without anything on the left side of that seat to stop him from moving toward the door bars and toward the left side of the car.
That's when the shoulder harnesses really kind of just broke the collarbones and did that damage.
But he moved far enough to actually hit his head on the wall briefly, get him a bad concussion, and somehow broke his jaw, which I'm maybe guessing that he hit his jaw or something on the door top.
But, you know, just a lot more injuries than I had ever considered.
