And sometimes I know the answer and sometimes I say, well, I'm not sure, let's look it up.

You know, so I love that variety about the job, but the core is always at the end.

How can I ease the suffering?

And that's why these debates or discussions about whether we stick with the DSM or whether we put it all in the bin,

don't bother me at all.

I'm fine with either as long as they're helping, as long as what we're doing is helping.

And so I have some patients who for them, a diagnosis of some sort is profoundly transformative and helpful.

And I have other patients for whom a diagnosis could be really dangerous because it will become locked in and it will become self-fulfilling.

And I don't always know the answer.

And so I have to explore it with them.

And that means the patient has to take on board a little bit of responsibility for that flexibility.

So I have to gauge how much I can be really candid and

When I approach, I often say, well, we could do this or we could do that.

And I cast a line into their life and reel it in to get a sense of what their preference is.

And as you pointed out, suffering comes from all sorts of things.

It's usually from loss, sometimes from pain, actual pain.

It's usually from loss of either grief or loss of independence or fear of independence, fear for the future, fear of death.
