The harder decision is in patients who have a consistent detriment in quality of life and people who at some point feel like,

I don't want this.
And these are patients who might be awake, who might be conversational.
And these can be really, really hard decisions.

And I support patients to the nth degree as far as this is concerned, because I don't think that under any circumstance we should be putting technology in patients that they don't have the right to turn off.
Because if that is the case, then we shouldn't be putting it in the first place.
So I feel fairly strongly about that because I think that we cannot expect a patient to make that type of life contract.
Where if you get this device under some type of arbitrary metric, we can decide or not whether you as a person have the right to stop a medical technology.
Can you imagine if we did the same thing for ventilators?
Thank God we don't.
I think the one thing that is useful here is, and I have, as a fellow, I, you know, there are a lot of stressful situations that I was in.
And this was one of the most stressful ones.
And I would seek them out because if I knew that some LVAD is being deactivated for one of our patients, I would go out of my way to be there.
And so there are a lot of things I've learned during that experience.
One is that I do think that it is really, if you have an LVAD patient and they have approached a phase where they want to have that device deactivated, I really think it should happen in the hospital, not at home.
