Well, eliminate vulnerabilities.
It always starts with good documentation.
And with that, medical necessity is paramount.
And establishing that in the documentation that the treatment provided was reasonable and necessary.
So that means including details like complexity, you're thinking about it, analysis and a risk assessment.
What are the options?
What are the alternatives?
We've seen a lot of what I consider excessive templates and check boxes creeping into documentation and often to the detriment of really telling the patient's story.
