And so it's not that we can't um then look at the effectiveness, but you do wonder like what am I really concluding about this?
Um, not to wade into the ACP controversy.
I definitely don't want to do that today.
But um
Adherence to the intervention is more flexible.
You might support the team.
You're there for um check-ins, but you're not, you know, training on fidelity and high you it's just not quite uh the other thing to and outcomes are collected in the course of routine care.
You're usually not soliciting
you know, thousands of patient reported outcomes, you're you're collecting for the EHR, what the clinic already collects from databases, admin, claims.
That's purely pragmatic.
And then there's purely explanatory.
You know, I'm controlling everything, you know, your placebo versus um uh medication trials.
The real the real research is in the middle.
It's really hard to design a pragmatic trial that's truly pragmatic in every domain.
And it's pretty hard to have an explanatory trial in palliative care that isn't slightly pragmatic somewhere.
So it's actually a spectrum, but we kind of talk about them though like they're vastly different.
I know, I know.
It's you know, maybe we can call it personalized medicine.
That's a new new hot term, right?
Um, this has just been at the foundation of my research since I started in palliative care.
