Hours before a deadly explosion ripped through a nursing home in Pennsylvania last month, staffers were worried about the smell of natural gas on several floors and brought in workers from the local utility company to check it.
The preliminary finding from the National Transportation Safety Board details how utility workers were on the scene for three hours before the blast, and it finds a worker traced a leak to a valve in a meter set in the basement boiler room.
Two residents and one employee were killed in the blast and about 20 others were hurt.
