# 72 Jicht, urinezuur en cardiovasculair risico, medicatie tegen jicht

In deze podcast wordt besproken hoe jicht (een kristallopathie) ontstaat uit een verhoogde concentratie van urinezuur. Urinezuur kan boven een bepaalde concentratie (risico boven de 0,35 mmol/L) niet meer volledig oplossen en slaat neer. Dit geeeft een ontsteking aan de gewrichten, roodheid en enorm veel pijn. Een jichtaanval ontstaat acuut, binnen 24 uur, meestal in het MTP 1 gewricht (middenvoetsbeentje en grote teen). Medicatie bestaat uit aanvalsbehandeling (colchicine, prednisolon of een NSAID) of preventieve behandeling (colchicine in een lage dosering), verminderen van de aanmaak urinezuur, door remming van xanthine oxidase (allopurinol en febuxostat) of verhogen van de uitscheiding van urinezuur via de nier (benzbromaron, probenecide, losartan en SGLT-2 remmers). Van de laatste categorie is alleen benzbromaron geregistreerd in Nederland.
