How are GLP‑1 drugs changing plastic surgery and creating a 'dark side' of rapid weight loss?

Dr. Kristy Hamilton 0:00
It's revolutionized plastic surgery not only in my practice but across the board. We have never seen so many patients successfully lose weight. Now everyone is coming in, they're losing more weight than they necessarily want to. The dark side. The dark side. It's having implications for places that we may not want to lose volume.
Dr. Gabrielle Lyon 0:21
What I'm hearing you say is that GLP-1s have revolutionized patient results, but also there's a secondary effect. But without any type of excess fat, what's happening?
Dr. Kristy Hamilton 0:33
Patients will come in, maybe they've already had liposuction or now maybe they've lost all of their fat. Now what we're doing is we're pulling products from these cadaver fat. So there's this product called Aloe Clay. So it is cadaver fat. It's been processed. It's been irradiated. And because it's been washed of DNA, you can inject it into other people and not reject it. Do you think that we're getting back to the more natural era. These things are under study and the great news is that all of our studies show that.
Dr. Gabrielle Lyon 1:13
The GLP-1 revolution didn't just change internal medicine, it changed plastic surgery. We have millions of people who've lost 50, 75, even 100 pounds on these medications. And a significant number of them are sitting in plastic surgeons' offices with a whole new set of problems. Skin that didn't shrink, glutes that have lost their shape, breasts that have deflated, and bodies that are now lean enough that there's no donor fat left for procedures that used to solve these problems. Dr. Christy Hamilton is a board-certified plastic surgeon in Houston who's been operating at the front edge of these shifts. She has invented a new procedure, the Miami Thong Lift, in direct response to what GLP-1 patients were asking for.
Dr. Gabrielle Lyon 2:08
She is one of only 32 surgeons in the US trained in a technique that lets women who lift return to the gym in just one week after breast augmentation. And she is injecting cadaver fat, processed cadaver fat, yep, you heard that right, into patients who no longer have any of their own to spare. She is a phenomenal plastic surgeon.

Why does rapid GLP‑1 weight loss cause facial volume loss and visible skin laxity?

Dr. Gabrielle Lyon 2:34
This is what the new era of plastic surgery looks like. Dr. Christy Hamilton, board-certified plastic surgeon, very dear friend, and I'd like to think of you as the resident expert plastic surgeon in the atmosphere and ecosystem of The Dr. Gabrielle Line Show. Welcome back. Delighted to be here. Since you were here last... GLP-1s have come on the scene and completely exploded. You're hearing a lot of GLP-1 face, GLP-1 bottom or butt, and just everything.
Dr. Kristy Hamilton 3:13
Everything. Has it changed practice? It's revolutionized plastic surgery, not only in my practice, but across the board. We have never seen so many patients successfully lose weight and keep it off. Wow. So patients would come in all the time and we'd say, you might not be a surgical candidate. You want the tummy tuck. You want these operations. But we have to get you at a safe BMI, a safe body habitus before we can do these things. Now everyone is coming in. They're losing more weight than they necessarily want to. And it's having implications for the dark side. It's having implications for places that we may not want to lose volume, like the face.
Dr. Gabrielle Lyon 3:56
Okay. What I'm hearing you say is that GLP-1s have revolutionized patient results, but also there's a secondary effect. Like, I mean, now liposuction might not necessarily be such a thing. Is it? And or if patients are losing so much weight, you do fat transfers. I've heard of fat transfers for the face instead of filler, right? Fat transfers. Also for the breasts, probably for the butt. There's probably all kinds of places. But without any type of excess fat, what's happening?
Dr. Kristy Hamilton 4:35
Yeah. So we've lost some of these reservoirs. And so patients will come in, maybe they've already had liposuction, or now maybe they've lost all of their fat. And a guiding principle in plastic surgery is replace like with like. So ideally, we can use fat if fat in the face has been lost. Now what we're doing is we're pulling from these cadaver fat products, which is interesting.
