And so what we really want to do is actually be saying, Well, what's the evidence for this procedure?
Who does it apply to?
What do they want and how do we give it to them and ensure they get it?
One of the issues is making sure the evidence gets into practice.
I think to a large extent the conversation at the moment is more in the m in the medical profession, but I think one of the th real issues we need to be addressing is ensuring that the community is much more able to make decisions about their care.
So things like decision aids, which allow you to say, well, okay, I've got uh bad arthritis of my knee, do I need a knee replacement?
What are the risks, what are the benefits, what are the options?
Getting more of the evidence.
It's interesting in the UK they've got patient reported outcome measures.
So in fact that's a great way of working out whether patients do benefit from procedures that we think automatically they might.
I mean that's a perfect example where you can have shared decision making.
There are a number of options and you can have a conversation around it.
But from the patient perspective, when they walk in, they don't necessarily know what are the options.
decision aids are being developed in different parts of the world and then certainly in the UK they're doing and uh Canada doing some work on this and you can actually put down on paper visually the sorts of options and the risks and benefits of those options.
So for example, if you want to look at harm and benefit you might have you know a hundred s faces and ninety of them might smile and s ten of them might be uh sad faces, indicating
that ninety people out of a hundred will benefit and ten people might not.
And so there are ways of actually articulating what is really a people perceived to be a very complex conversation, but really
can be made much more easy to work through.
And as you say, you can actually say say to somebody, Well, it's not just knee replacement, you're a bit overweight, you might want to actually go and lose some weight to get a bit of pressure off those knees.
Or as you say, you might want to go and have some physio.
