So you need to be focused on the moment at hand.
And so when you're talking about what could go wrong, yes, before a surgery, I go through my mind, what could go wrong?
And what are my plans?
What are the steps to go through if a vessel is bleeding and I can't control the bleeding?
What would I need to do next?
We'll start with try to control the bleeding, compress it, order blood, maybe call in a second set of eyes and hands to help me.
Boom, boom, boom.
I have a set thing I'm thinking about.
If a nerve looks like it's damaged, what am I going to do?
Can I consider repairing it?
Can I suture it?
All those things need to be in my mind before the surgery.
But the second that surgery starts, I am focused on the next step.
Whatever it is, the next step, the next step, the next step.
Nothing before, nothing after.
So the first moment that you realize something has gone wrong or that you've made a mistake is reject your first reaction to it.
And I talk about that in a protocol called IRISE in my book.
You want to reject what your first impulse is going to be because it's almost always self-preservation.
So you need to reject that initial thought.
I talk about in the book, I talk about a time when I'm putting a scope through the hole and into the brain of a child and we encounter severe bleeding.
