Important point and I wish that there were
Easier ways to get that message out to parents because it doesn't seem like that to parents, you know.
Um, the American Academy of Pediatrics, as you well know, says titrate to the maximum dose tolerable with tolerable side effects.
Um but what often happens, I believe, in practice is that
You know, parents will try one stimulant and they'll be unhappy with the side effects and so they will switch to a different stimulant and then a different stimulant without titrating and really seeing what the effects are.
Is that what you've found sometimes to be true in practice?
I sometimes right context will joke around that I was very fortunate um to have a child who had such severe ADHD that we really didn't have a lot of um time or option to consider whether we were going to use a stimulant.
You know, this was my child who at two was
Literally climbing the furniture um in the preschool classroom, jumping off six foot bookcases, you know, wrestling with other children, um, jumping out of shopping carts and so
You know, the blessing of a severe ADHD is that you recognize very early that you don't have time to spend thinking about
Should we try medicine?
Um, and so, you know, given that he's 18 now and has had very good outcomes, I guess he's a case study that's consistent with what research seems to be indicating, which is that there are strong long-term positive outcomes for stimulant medication.
