They completely miscalculated.

But the root cause of the daily evil that the Iraqis go through cannot be put on the broad shoulders of the Americans because then that removes the personal agency of the actors in their daily lives that are causing them all the pain.

But there is a reflex, and dare I say, forgive me, a suicidally empathetic reflex that

I'm right.

So it's not they made an error.

They knew it was.
