I think the thing to take away from this-
is daily activity.
I think what you'll find in a lot of these states is people are relatively active for the most part, either because the culture of the state encourages it or because of the way the cities are designed.
In fact, big cities where it's inconvenient to drive tend to have better health, even though their air quality is so bad.

I don't know how many people are in there to make that big.
