One of the major problems that I saw was this issue around disenrollment.
Um so um
my patients were getting uh we talked about this on our last podcast, um, getting kicked out of hospice.
Um, uh I what I call expelled from hospice.
Um, some people call graduating from hospice.
Um and it c goes back to this issue around the hospice model.
Not really being uh
Uh modeled around the needs of people with dementia.
It's really set up as a model to care for people with metastatic cancer.
One of the eligibility criteria for hospice is that you need to have a terminal illness with a prognosis of less than six months if the disease runs its likely course.
And this is something that's very difficult to determine in people with dementia.
So what happens is that people end up they come on, they look like they're very ill, but then they just they don't they don't necessarily die within six months and it's very challenging to determine when they will die.
And so
They don't they no longer meet
criteria for hospice and then they need to get disenrolled.
Um, so I saw that as a major problem um because when people get disenrolled from hospice, um, they lose all of those interdisciplinary palliative care services that hospice was offering, you know, nurse case management, hosp uh social worker case management, medications.
Uh
medical equipment, a home health aid, um
All that stuff goes away.
Um, and I just want to highlight some work.
