Do you talk about the impossibility of the ordinary person to find a good enough doctor
When I challenge specialists about this and I say to cardiac surgeons, for example, you know, how do you choose a good cardiac surgeon?
Knowing full well that they know exactly who they'd go to, they say, Oh, we've got a cardiac surgery register in Australia and when we analyze the figures, there's very little variation between cardiac surgeons.
We're all pretty good.
It doesn't really matter who you go to, except their own behavior gives the lie to that.
They know exactly where the
differences.
And if you talk to intensive care specialists about cardiac surgeons, just to choose cardiac surgeons as an example among many, they know exactly who they'd go to.
They can see the differences.
Coming back to something you alluded to a moment ago, to what extent do you believe the club culture, to use Ian Kennedy, the British ethicist and lawyer phrase and which you use in your book, the club culture still exists in medicine.
To what extent does the club put the wagons into a circle and protect their own?
there's an obligation now, I think, in Australia under the new registration system in Australia to if you know a colleagues impaired, you actually have to tell the authorities.
And are you focusing on the five percent or are you wanting to make a good enough doctor on average a better doctor?
So good enough becomes a higher standard over the years.
So you're shifting the whole population of doctors to a better place or are you focusing on the five percent and making that one percent or half percent?
Well that's right.
I mean you're arguing for recertification.
And I must s I must say, all these years on, I still have the odd nightmare once or twice a year that I haven't taken my final exams yet and I've still got to take them.
And I wake up in a cold sweat.
And here this bloody lawyer from New Zealand comes along and says and would say, Well, I've got to take an exam again every so often during my career.
