With OHCM, because of the thickening of the anatomy, there's a pressure gradient that cardiologists can monitor and use as a touchstone for titration of a medicine for the treatment of HCM.
You don't have that in NHCM.
Okay.
You do have probably equal severity of symptoms to your question, but it manifests differently.
And absent a pressure gradient, I think physicians are looking for other ways to manage these patients.
That's where I think the ACASIA study may hopefully be a seminal study for the use of a cardiac myosin inhibitor in these patients.
We've designed and have conducted a study called ACACIA that's going to read out in the second quarter.
And based on things we know, and we have caucused experts in NHCM over several years.
In fact, we hired several of them to design and oversee the conduct of this study.
We believe we've enrolled the right patients in the right centers for this experiment, which is to say, does a cardiac myosin inhibitor offer a treatment option for these patients where right now they have no approved treatment?
BMS conducted a study called Odyssey of Camxios in a similar population.
And that study, unfortunately, failed, but it was a near miss.
And we think it was encouraging to the therapeutic hypothesis.
And we believe that Acacia was designed and the conduct of that trial has us optimistic that we may see a positive study in Q2.
So it's both a risk and what could hopefully be a telltale sign of symptom burden relief, which KCCQ has consistently, both in obstructive and in our phase two non-obstructive, been one of those early harbingers of treatment efficacy.
We've seen over and over and over now where KCCQ, which has a high fidelity read to patient symptoms, is where Afikampton or mycorrhizal shows up at its best.
In phase two, we saw outsized
treatment efficacy with aficamtin, albeit in an open-label fashion, but even controlling for historical placebo event rates, we saw very large magnitude efficacy of aficamtin in patients with NHCM.
We've consulted with
the gentleman who has designed and has been implementing KCCQ over decades and with FDA, where there's a high test-retest reliability and a pretty well-known and calibrated
