I'd quibble slightly with the idea that this is to do with human error.
I think there is a human mistake in it, but I think it's a systemic error.

The best example, and Dale will know this, do you know the hair-pulling thing

Hair-pulling last season was a straight red.
And you can think that was harsh and a little bit kind of excessive, and you can think that football does occasionally have a weird desire to police things that don't actually do any physical harm at the same time as allowing stuff that does do actual physical harm to go a little bit less, you know, to be punished a bit more lightly.
But hair pulling to me anyway, and this is personal, feels unsportsmanlike at the very best.
