And in this program, our initial evaluation of the program, we evaluated the effect of patients who we randomized patients to receive the STAR program or usual care.
And we evaluated the effect on a combination of readmission and mortality at 30 days.
Um, and so in the initial clinical trial, we find we found about a five percent reduction in readmission and mortality.
For patients who had support through the STAR program versus patients who had uh the usual care.
We did a uh longer follow-up, so that was at 30 days.
At 12 months, that difference uh was sustained.
Um, and patients who received support through the STAR program had about a 7% reduction in readmission and mortality compared to patients who uh received usual care.
And I'll point out that even though we were so excited.
That this program had benefit.
When you look at the absolute numbers, and this lines up with some of Carolyn's reports and things, the percentage of patients who were either readmitted to the hospital or died at a month was 65%, even in the intervention group.
So these are still patients who have really high uh adverse outcomes, even in kind of an optimized intervention.
group.
And then people ask me a lot, what what exactly did the intervention do?
What did the star navigators do?
And so we went through and did sort of almost a qualitative analysis of all of their notes and really drew out the different components of care that they were doing and the frequency with which those components were being delivered.
And as you can see the
These these navigators are really just i incredible unicorns that can do almost anything and really provided a tailored set of care to patients depending on what they needed.
So the most common thing that they did was care coordination.
So they would help patients arrange follow-up or uh reschedule visits if they needed to or those type of things.
Um, there was a lot of health promotion counseling, so lifestyle, exercise, smoking cessation, um, reminders to take their medicines or encouragement to take their medicine, emotional listening, uh, which is not something that we necessarily design to be part of the program, but I think aligns with some of uh Joe McPeak's work, um, that a lot of the role of these navigators was just being
