And they had what the pilots call got to get there-itis, where you tend to minimize things that prevent you from getting there, and you maximize things that support you getting there.
It's a common thing.
I mean, it's not anything...
I mean, you look at accident reports for aircraft, and it's usually, in a lot of cases, that type of thinking has something to do with the decision making.
So it's nothing NASA particularly has an issue with.
It's across the industry, and it's just something we have to realize is there, and we have to try to control it.
We call it the Swiss cheese effect.
To have an accident, you have to line up multiple holes in the Swiss cheese.
You take slices of cheese, and it usually takes three, four, five, as many as 10 of the holes lining up to get an accident.
Because anything less than whatever the minimum is, the safeguards kick in, and you're saved.
But you line them all up, and you're just asking for trouble.
