welcome back clinical problem solvers this is another episode of the space learning series sls with my amazing team i am vale calling from chile san francisco this morning and uh let's introduce the team jazz how are you i'm good i would say i'm very tired june's been a very busy month for me and still trying to balance um trying to be a full-time attending and then also taking care of my kids so
I just want to balance out all this tired feeling saying that June is pride.

Okay, so this is the case of a 40-year-old right-handed woman that presented to the ED with confusion and altered speech.
She noticed earlier in the day of the presentation a diffuse headache and also neck pain of moderate intensity.
Two hours after the onset of the headache, she noticed a sudden onset of decreased sensation over her right arm and her partner also noticed that she had trouble speaking and described it as if she had a hard time getting words out.
After this, she went to a local hospital that was three hours outside of the city where I was working at, and they did a non-con CT scan that wasn't remarkable.
And while they were transporting her to a center that could provide a stroke care, which was like their leading hypothesis at this time, she became increasingly agitated and combative.
And when she arrived at our institution, she could no longer follow commands.
Shortly thereafter she became comatose and was intubated.

Okay, so I'm gonna give you a little bit more information.
So when we questioned her partner on arrival, they said that she had given birth to a healthy baby five months earlier, and that there had been no complications during or immediately after the birth.
She also had recently had an episode of an upper respiratory infection and otitis media, for which she had been receiving amoxicillin clavulanic acid the week previous to presentation.
So when she arrived, her vitals were a temperature of 102.2 Fahrenheit,
a heart rate of 98 beats per minute, a respiratory rate of 12, and she was intubated at this point, and she was at rating normal.
On examination, she was comatose with a Glasgow Coma Scale of 6.
She could not open her eyes, even with noxious stimuli.
