She had no verbal response, but she did have some movement with pain.
On atopic examination, she had mild ear canal redness, but there was no fluid or any changes in the tympanic membrane as bulging or opacification.
The neuro exam was limited because of her comatose state, but as mentioned, she had no eye opening, even to painful stimuli, no verbal response, and there was some recall with painful stimuli on the left side, but none on the right side of her body.

Her basic labs showed hemoglobin, WBC, and platelets that were all within normal range.
The electrolytes and rhythm function panel was also normal, and her blood cultures were drawn and an LP was performed.
Her opening pressure was 50 centimeters of water that, you know, usually the opening pressure has an upper level that's normal of 25.
WBCs on the CSF were 3,710 with neutrophilic predominance.
Total protein was 182.
The normal ranges go from 15 to 45 usually.
And the glucose was 1.

Well, she at this point, actually before the LP, she was started on antibiotics, as just mentioned, and she also started on dexamethasone because as you have emphasized.
So over the next 24 hours, she improved dramatically with antibiotics and dexamethasone and was able to be excavated.
She was now awake, alert, and oriented times three, but she still could not follow two-step commands.
She was able to name objects and could repeat, but her speech was altered in a sense that she spoke in short, almost like cut phrases.
