I think something we didn't talk as much about today, but probably across the two episodes will come through is this idea of
You either treat the PTSD or the sleep.
And that's a question that I get asked a lot is, you know, which should I treat first?
This is an empirical question that I would love to do a randomized controlled trial for.
So if anybody has funding in a population, call me.
But we haven't done that.
That empirical work is not there yet to say which way we should structure treatment.
And so for some people, we think about doing the sleep treatment first because if we treat the sleep, maybe the PTSD will resolve or come down.
or we need to get people sleeping enough that they can do the cognitive work of PTSD.
Some people will argue that you have to treat the PTSD first.
And if you do, the sleep problems will resolve.
But we know that's not true from the research literature and from our experience.
The two things that persist even after a successful course of treatment for PTSD.

And so I really think about this idea of
The best treatment is the one that the patient is willing to do.
And so when somebody comes in to me that has sleep problems and PTSD, I present all the options.
And I say, you know, which one do you want to start with?
Where would you like to start?
And then we'll reevaluate because maybe treating one does address both.
