Yes, and getting people to understand that it some takes a couple of days, but it doesn't take weeks or months to determine if someone's having a response to a particular medication.

And then thinking about our research studies where typically um every week the dose is increased until they're much better.

But part of it is the way our healthcare system is organized.

Um and um

You know, I I don't think it's a good model where someone's getting a pres one prescription and then just told to call back if there's problems or come back in six months or or or yeah, or sh or a different period of time.

But the titration is really you need to have intense contact um during that those couple of days.

Um, and I I guess the other thing in terms of treatment is how do you sequence when do you start when do you consider medicine and when do you consider behavior therapy and when do you consider the combination?

Uh and I I think my belief is that it's best in children to start with behavioral interventions first unless the ADHD is really severe.

And a lot of a lot also depends on age.

Um, but um that way uh if you do everything at once, you don't know what's really working and it it's combined.

Um so um and I think the recognition is that medicine helps the core symptoms, but doesn't really teach skills.

And that's where, you know, the psychoeducation and the coaching and all those things take place.

Yeah.

Well a couple of things that um uh I guess I'd like to mention.

Um I think um

It was a big change, you know, ten or fifteen years ago when we said that AHD is not a short adjustment period, but a chronic disorder.

And I think now with uh some of the merging research, it's a chronic but fluctuating disorder.

And it really depends on um the context and the the time um and and where someone is in their lifespan.
