There are certain periods of time, critical periods, where ADHD's really significant.
Um, you know, kindergarten, fourth grade, first year of high school, first year of college, you know, those are like those critical times.
Um, but then there are other times uh when people are um have gotten through school, for example, um, that ADHD might be less of a problem.
And so
I think it's in you have to invest the time early on when it's first diagnosed uh to develop a treatment plan, but then you have to have regular reassessment of what you're tr what you're treating.
Um the other change in conceptualization is that uh I I think now we tend to think of ADHD more as a twenty four hour disorder.
Um when I started in this field, um, you know, in the eighties, the typical treatment was Ritalin twice a day.
And that's because we thought it was a school disorder.
And we thought, well, if kids can sit still in school, they're gonna do well.
Um and then uh through research and understanding of ADHD, uh following them up, adolescents didn't get better on their own, even though the hyperactivity may be less visible.
Um, they still had attention problems and were at great risk because of their impulsivity.
Why would we why would we suggest taking a medicine that only lasts eight hours?
Um and so research suggests that you could treat ADHD for longer periods of time.
And maybe they do better in terms of driving or in terms of their um socializing.
Um but maybe they're not sleeping or maybe they're not growing.
Um and then there that you get into this cycle of um being chronically tired or chronically sleep deprived.
Um so again, realizing that you have to look at the twenty-four hour picture.
And maybe we don't need the most effective stimulant for the day if we can have a regimen that helps them sleep at night.
And of course, all through treatment, teaching them good sleep sleep hygiene.
And we don't want to underemphasize the importance of lifestyle factors, um, especially uh physical activity and exercise, which is a a huge, huge
