What I realized is, well, whose job is that?
We don't have any kind of insurance program, whether it's through Medicaid and Medicare private insurance, maybe a little bit with long-term care insurance, which is extremely expensive.
We don't have the diaper changing option or the benefit.
We don't have the laundry benefit.
We don't have the clean out your fridge benefit.
Who's doing all of that stuff while this caregiver is trying to manage the nebulizer, manage the morphine, comfort, manage the people coming in for little visits here and there?
As a palliative care doctor, what I realized, and I'd started to realize it even before the film, but this film really made me realize it, is that I was idealizing what hospice can do.
And it's not what they do.
Hospice is an amazing, amazing service.
Bambi could never have been home without hospice.
And that's where she wanted to be.
But they are paid about $150 a day to manage all of the durable medical equipment, all of the medications.
Anything that comes up that is serious symptom management.
And that takes a lot of effort and time.
And their purview is not the household and hygiene.
And those things are very expensive to pay for.
If you're lucky enough to qualify for IHSS, which means, quite honestly, that you're unlucky enough to have a low income to be qualified for Medicaid...
then you might get a little bit of time allotted through the IHSS benefit in home support services that Medicaid provides, but that's minimum wage.
And so you can't then go out and hire someone else to come in and do that stuff.
So that money, if it comes at all, is really for that caregiver to support them in all of the financial outlay that a caregiver has to do on top of it.
