So so you know, every I think a lot of the um the the uh different things that, you know, I mentioned resources, policies, practices, res um

Those are all decisions that um hospitals make, that leaders make in hospitals.

And so that is what I mean by investment.

You know, whether what what are hospitals sort of saying these are priorities that we want to invest in to support doctors, or this is the direction that we want to go to support doctors, or and I think oftentimes they align with, you know, culture hospital culture, values, ethics, and that sort of thing.

So so actually like.

I think the intersect uh interaction between the policies and structures, et cetera, that a hospital has influences the culture, which then influences the policies.

So yes, um, there is a default of um, you know, high intensity care in American medicine.

And and that that is something that the paper showed, but that's also something that many other um, you know, papers and and data in the US has also shown.

Um, if I were to summarize this better, it's that there's like

Institutional structures that either thwart

or facilitate a physician's ability to um go against that default of high intensity care.

Yeah, so so you know, that's that's what I mean by um factors that thwart or either encourage um a default of in uh high intensity care.

What came up in a lot of the interviews was that I think generally clinicians um, you know, don't want to be giving high intensity care that's not going to help people near the end of life.
