Are you finding differences in operative rates?
Which means that there are a whole lot of people
out there not being operated on who are dying early because they missed out on the opportunity because they went to an inexperienced institution surgeon.
So you don't know yet whether or not the rates of surgery are different.
You know if you've got the denominator of numbers of people with pancreatic cancer and they go to hospital X, a lower percentage are being operated on.
When you bring this up, surgeons always say, Well, I know of a country hospital where they're getting fantastic a fantastically devoted surgeon because nobody goes to work wanting to do a bad job gets great results.
Western Australia is a good example for pancreatic cancer.
They don't actually have a large volume, but they do get very good results in one particular hospital or all Perth.
So there are exceptions to this.
What I really meant by that was whose responsibility is this to change it?
If you're a private hospital operator, if you're you're running a public hospital, is it the surgeon who should take his or her patients to a new hospital?

But I hear that some of those hospitals where they put their hands up, the surgeons don't want to give up their cases.
They say, look, these cases make my life interesting.
When one of these comes along, it's a challenge and that's what keeps me going.
So if somebody's unfortunate enough to develop esophageal pancreatic cancer and is listening to this, the key questions to ask are how many operations does your institution do a year?
And if it sounds like a low number, have a discussion about somewhere that's a high number, but you're not able to say what the threshold number is.
And am I going to be diagnosed by a team and treated by a team?
