Well, I I again it it it um uh uh it's a treatment decision you make in terms of how to sequence treatment and thinking about it not so much in just for the child but the whole family system.

So um recognizing, you know, when you see let seven year old with ADHD that's coming in for the evaluation.

And they're talking about the difficulties the child is having um at home.

Uh getting ready for school, getting ready for bed, mealtime.

Um and then as part of the evaluation, you're getting a family history.

And you might find um that one of the parents had similar difficulties as a child, and they still do.

Um they're always late for the appointment.

Uh and so you're not gonna be very successful in treating the child if the parent's ADHD either interferes with treatment or the parent has another psychiatric disorder like depression.

I I kind of joke, but the typical family that comes to the the my clinic is one where the child has ADHD, um, one parent has ADHD, the other parent has anxiety, and the kid has a little bit of both.

And then you're trying to figure out what to treat.

Um but I think you should broaden the question not just what to treat in the child but what to treat in the family system.

So behavioral parent training is not going to be as effective if the parent isn't able to follow homework assignments.

Um, medication trial is not gonna be effective if the parent can't

Fill the prescription, make the appointments, monitor their sleep and their weight.

So you always kind of make an assessment of what's the biggest problem or where can I be most successful in treating it.

And you always want to um

uh, you know, get I guess have a suspicion or look for ADHD uh or other things that could interf get that could affect the child's treatment plan.

Now

When we treat a child with ADHD, it's not typically the most effective treatments aren't with the child in individual office.
