The way I understand it is that
And there's some good data showing that this is too simplistic of what I'm about to say.
But the way that I have always understood it is that while your bones look good on DEXA and on imaging when you're on bisphosphonates, the bone integrity itself is not as good.
It's more chalky than it is healthy.
And so the more recent studies that I've looked at show good activity of osteoblasts, more normalized activity than you would think of the osteoblast-osteoclast balance, and that
You couldn't possibly have just chalky, dense bone that's worthless if these cells are showing such activity.
I'm still not a huge fan of them.

I would not go on them.
I would do squats and deadlifts and whatever else I had to do in the gym all day long before I would go on a bisphosphonate.
And that's the part about bisphosphonates that aggravate me is that they are considered first line when I don't know why strength training is not first line, period, period.
Period, period.
If you can take HRT, I don't know why strength training and estrogen replacement therapy are not first-line therapy for every single woman in America, and the world for that matter, before we would reach for a pharmaceutical.
I think that that's kind of first-line therapy for everything, if you ask me.
If you're not strength training, then you are missing the benefits of so many things.
So your bone health, we know this to be true, and there's lots of great docs finally talking about it.
